National Lab Loyalty Card Membership

We are proud to introduce the National Lab Loyalty Card Program—designed to offer exclusive benefits and added convenience to our valued patients. Loyalty Card holders can enjoy special discounts on diagnostic tests at our main branch and select facilities.

 

 

How to Get Your Loyalty Card

You can obtain your personalized Loyalty Card by paying a one-time fee of Rs. 500. Once your request is processed, the card will be mailed to your registered address within 14 business days.

To apply, kindly provide the following details to our representative:

  • Full Name

  • CNIC Number

  • Contact Number

  • Residential Address

Loyalty Card Benefits

  • Flat 25% Discount on all laboratory investigations conducted at National Lab Head Office.

Card Validity & Renewal

  • The Loyalty Card is valid for 1 year from the date of issuance.

  • It can be renewed at any time for just Rs. 300.

  • In case of loss, please contact us to request a replacement card (additional fee applies).

Terms & Conditions

  • The Loyalty Card is non-transferable and may only be used by the registered cardholder.

  • The card is valid for one year from the issuance date and can be renewed upon request.

  • Discounts at partner healthcare providers (outside of National Lab) are solely at the discretion of those providers.

  • National Lab holds no liability if an external provider discontinues their participation in the program.

  • If your card is lost, please report it immediately to receive a replacement (subject to a fee).

  • National Lab reserves the right to modify or discontinue the Loyalty Card program or individual cards at its sole discretion.
